Harrington Family Health Center offers a sliding fee schedule. Click here to download and print the application.
In preparation for your first visit to Harrington Family Health Care, please observe the following guidelines.

  1. Arrive 25 minutes early to fill out forms.
  2. Bring your insurance, Medicare, or Maine Care card. If you are registering for sliding fee click here to download and print the application form. Fill out the form and bring it with you to your appointment.
  3. Give 24 hours or more notice if you cannot keep your scheduled appointment.
  4. Payment is due at time of service. Insurance coverage benefits vary; your portion is due at the time of your appointment.
